ABOUT CIRCLE HOME INC

Circle Home Inc offers a program of telehealth services focused on helping moderate to high-risk patients (primarily those covered by Medicare) living at home to better manage their care so they can minimize or avoid breakdowns in their health resulting in expensive hospitalizations, emergency department use or other institutional care. The Circle Home Program (CHP) is carried out through a highly specialized, multi-state physician group practice specializing in remote chronic care coordination and monitoring of patients in the home. Care coordination and health status monitoring activities are carried out by a team of nurse practitioners, registered nurses, social workers, and dietitians.

Director of Physician Engagement

The purpose of this role is to recruit and expand the physician network that supports Circle Home Telehealth Services (CHTS), a company that remotely monitors and coordinates care for complex, vulnerable patients living at home who are covered by traditional Medicare. This entails explaining how the program operates to physician offices, closing contracts with these physicians and assisting them with the ongoing identification of eligible patients in their practice who could benefit from CHTS services.

This role also seeks to identify and develop relationships with third party organizations that serve the Circle Home target population with related services such as home care and device companies.

The geographic focus of the position is the State of Maryland. The position involves direct personal interaction with providers and their staffs - both in person and via telecommunications. The successful candidate will have:

  • Existing business relationships with physician practices in the geographic area through prior or current work experience and be credibly able to develop an ongoing provider relationships using these prior relationships as “door openers”.
  • The ability to identify potentially interested physicians through networking, cold calling, and market research and then “close the deal” to have the physician join the CHTS network.
  • Expertise in creating and delivering compelling presentations to prospective physicians.
  • Experience in care coordination for patients and their challenges – including clinical, social, behavioral, economic - that vulnerable multi-chronic patients face while living at home.
  • In-depth working knowledge of primary care office operations and a firsthand understanding of the demands on the staff in these practices.
  • The capability to credibly build a community of interest among the physicians who participate with CHTS so that shared experiences can benefit and strengthen the CHTS Program.
  • Experience with various EMR systems and the operational/technical know-how to identify potentially eligible patients for CHTS services based on data in the native EMR system used by each practice and then effectively follow-up on the pattern of referral results that emerges to assure appropriate referrals are being made.

  • Ability to work independently and as part of a team
  • Registered Nurse with current Maryland license preferred
